The World Is Digitising Care. England Is Still Arguing About the Wiring

A hospital in Lakeridge, Ontario, has just reached the highest global benchmark for electronic record maturity, the kind of accreditation that means a nurse can pull up a patient's imaging, medication history and allergy alerts on a single screen without a second login. In Delaware, ChristianaCare is completing a state-wide migration to one shared platform, so that a patient discharged in Wilmington and readmitted forty miles away arrives with their history already loaded. In Indiana, the US Department of Veterans Affairs has now extended its federal record system to more than 617,000 veterans, backed by an AI assistant fielding tens of thousands of clinician queries a system this size generates in the transition. None of this is glamorous. It is plumbing. But plumbing is precisely what the NHS has struggled to lay for the better part of two decades, and the contrast is not flattering.

England still runs on a patchwork. Some trusts operate mature electronic patient records, others limp along on paper-based workarounds bolted to ageing systems, and the promised Federated Data Platform, meant to knit hospital, community and primary care data together, has moved cautiously in the face of clinician suspicion and a public still wary after previous data-sharing schemes collapsed under scrutiny. The lesson from Ontario and Delaware is not that England lacks ambition. It is that single-system consolidation requires sustained political cover and money that survives more than one spending review, conditions that have rarely held in England's favour. Digital transformation in the NHS has too often been treated as a technology procurement exercise rather than a workforce and governance one, which is why systems arrive, underperform against expectations set at business-case stage, and get quietly reworked years later.

The Botswana example cuts differently and matters more than it might first appear. A ten-year agreement between the country's health ministry and the Gulf-backed AI firm M42 is building national genomics capability while simultaneously securing pharmaceutical supply chains through Mubadala Bio. This is sovereign capital doing what NHS England's own life sciences strategy has struggled to do at pace, tying health data infrastructure directly to industrial and supply chain resilience rather than treating them as separate workstreams. The UK has genomics strength through Genomics England and the life sciences sector remains a genuine national asset, but the funding model depends on a mixture of public grants and fragmented private investment that cannot match a single decade-long sovereign commitment. If Gulf capital continues moving into health infrastructure at this scale, UK policymakers will need to decide whether Britain competes for that investment or watches genomics and precision medicine capability concentrate elsewhere.

The DocGo acquisition of Hicuity Health, folding tele-ICU and remote nursing capacity into a mobile care platform through roughly fifty-two million dollars in assumed debt, points to where virtual care markets are heading: consolidation around providers who can move seamlessly between acute remote monitoring and home-based management. NHS integrated care boards have been experimenting with virtual wards for several years now, with mixed results and uneven investment across regions. A maturing commercial market in the United States, where acute and home care providers merge rather than partner loosely, offers a preview of the kind of vertically integrated virtual care operators that NHS commissioners will increasingly encounter as suppliers, and will need the procurement sophistication to evaluate properly.

None of these developments obliges NHS leaders to imitate them wholesale. Ontario's accreditation, Delaware's consolidation and Indiana's federal rollout all rest on governance structures and funding certainty that England's fragmented commissioning landscape does not replicate easily. But the direction of travel across three continents is unambiguous, and it is not about acquiring cleverer algorithms. It is about who controls the record, who can move a patient's data with them, and who has the industrial policy to back genomics and supply chains with more than a single parliamentary term's patience. England has the clinical talent and a national health data asset most countries would envy. What it has lacked is the sustained institutional will to finish what it starts.
